At KidZdent, our goal is to provide every child with a healthy, happy smile. Your child’s teeth are meant to last a lifetime, and we are committed to guiding both you and your child through every step of that journey. Our gentle, child-focused approach creates a positive dental experience that benefits your child for years to come.

Below are our New Patient Registration Forms, Parent/Guardian Consent Form, Insurance Billing Form, Media Release Form, X-ray Request Form, and HIPAA documentation. You will also find a questionnaire/symptom checker. Please complete all forms and bring them with you to your appointment. Please remember to bring:

• Completed New Patient Forms
• Your Insurance Card(s)

KidZdent is in-network with Delta Dental Premier, Cigna DPPO, and Aetna DMO (limitations apply). If you have a different dental insurance provider, we will be happy to verify your out-of-network benefits and provide an estimate of any potential out-ofpocket costs. Please note that benefits are determined once a claim has been submitted, and any out-of-pocket expenses are the member's responsibility.

If your child has had X-rays taken at a previous dental office, please have them forwarded to info@kidzdent.com before your visit. If X-rays are not received, KidZdent may need to take new ones, which could result in an out-of-pocket expense if not allowed by your insurance.
